Son of Antiochos III and Iotape, descendant of Seleukos I of Syria, he was the last king of Commagene. Antiochos IV spent his youth in Rome, at the court of Antonia Minor. He was probably close friends with emperor Caligula, who in AD 38 granted him the kingdom of Commagene, years after the death of his father. He married his sister Iotape I and remained on good terms with the later emperors Claudius, Nero and Vespasian, until he was implicated in an alleged rebellion against Rome, accused by the governor of Syria, Lucius Caesenius Paetus. Antiochos, faced with the advancing Roman army, decided to flee to Cilicia where he was arrested. Eventually Vespasian granted him and his family freedom, allowing him to return to Rome.
