The medallion displays a classic philanthropic scene: a central robed female figure symbolising Charity, flanked by two children, all framed within an ornate architectural canopy. Above is the red enamel shield of Lancashire, and surrounding the scene is the inscription: EAST LANCASHIRE MASONIC BENEVOLENT INSTITUTION The date 1925 appears on an enamel plaque at the base, highlighted by a small red Tudor rose.
